I came across this little box on the Dieline blog. There is a downloadable cutting/folding guide. It's not a template, but some of you hybrid genuises can probably make it work :)

Oh and just beware, that the template is at 72dpi. these are the kind of templates I'm used to working with from when I did computer crafts :)
