Default Pharmacy mix-up

Yikes! Jeff just went to pick up my lovenox prescription for the month. These are the blood-thinner shots I give myself every day during pregnancy. Normally, I know that I take 40mg daily, they come in a yellow box, and I get 30 pre-filled syringes. Well, Jeff brought the bag out, and I noticed it was much smaller than normal. There were only 12 syringes, and the box was blue. So, I went in to ask if there had been a mix-up or something.

Turns out, instead of giving me 30 at 40mg, they gave me 12 at 100mg! That would've been 2 1/2 times my normal dose if I'd taken those!!!

So, the pharmacist sorta freaked out, apologized all over herself, refunded our copay, and gave me my correct order.

Sorta scary! So, I guess the moral of the story here is to always double-check what medications they give you. If this had been my first time taking this and I didn't know what I was looking for, this could've been a bad thing.

Yikes! But you know what Libby it happens ALL the time. My mom has been on blood pressure medicine for about 16 years and I can't tell you how many times they have goofed up her prescription of that and a couple of other meds she takes. Just a month ago DH's thyroid script was wrong when we picked it up b/c of pharmacy error and if I hadn't checked it he would have gone right ahead and taken the wrong dosage.

It's so very important to always check and double check...and then I'm always paranoid too that there is a mislabel or something on the bottle.

yup. One time a dentist prescribed me an antibiotic for a tooth abscess. the label on the bottle was for the antibiotic. The actually PILLS were a cholesterol medicine and I had NO idea till 6 days later when the tooth abscess was raging even worse and I took the pills to the dentist. I'm SO glad you didn't take the shots and knew what you were supposed to have Libby ... that could have been disastrous!

That's the importance of trying to stick with one pharmacy too so they know your usual dose and question when there's a "change"

So scary and it's a good thing you knew your medication Libby. I always try and have my doctors go over exactly what they are giving me just in case there's ever an error. My thyroid meds used to get filled wrong constantly especially when the dr would change the dose pretty regularly.

Good grief. It's so terrifying to me to think of these situations happening all the time! I think about the poor elderly patients who take so many meds that they can't keep up with what's what. My grandma has them changing her meds so often that she'd never recognize if they gave her the wrong thing.

They gave me the wrong synthroid dose in the hospital when I had Kaylin - more than once. Luckily, with synthroid, it's not really going to hurt me too much if my dose is off a little bit here and there. But, yeah, it's a little bit scary. Synthroid comes in different colors for each dose - even the generic versions follow the same color coding - so it's easy to tell if you're getting the right dose. When I told the nurse that I was supposed to get a purple pill, not a pink one, she tried to tell me that sometimes the generics are different. Not true for Synthroid. It's a little scary when the patient understands the meds better than the people at the hospital.

Glad you noticed it and got it fixed Libby!

ugh... ditto what meg said about sticking to the same pharmacy. Mine is soooo good about keeping track of my coumadin. bc my stupid body still isn't 100% stable, I have 3 different scripts and theyre amazing, always double checking etc. calling me, refusing to give me the meds till i clarify (Which, may sound like a pita but i'm grateful for it. Od'ing on blood thinners is not exactly something you want happening )

Mix-ups aren't always common...The mail order pharmacy I work for had zero misfills last year and they fill thousands of scripts daily.

Libby, I'm sure you're shaken up! I'm SOOO glad you caught it!

Oh man so glad you caught that Libby!!! I guess we have been very fortunate to have never had this happen to us, but then again none of us are on "daily" meds or even go to the doc that often. I'm sure if we were getting meds regularly we probably would have experienced this!! I know now I'll always always double check out medicine!!!

I do know that when we do get our meds we get them at CVS and I've noticed that they list what the med should "look like" on the label (for example: this should be a white liquid) which would not catch all mistakes but I'm sure it helps caught some!!! Maybe I'll start looking up our Rx's online before I pic them up at the pharmacy from now on...just to arm myself with more knowledge!!!

there was one time we got the right meds for Asher, but the label was wrong... it said to take one and stick it in your vagina once a day... umm... yeah, what vagina LOL... we were upset at first but now can get a good laugh out of it... thankfully I knew what his meds were supposed to look and smell like and knew it was correct, just not the right label... so he didn't miss any while we went back to get it all straightened out.

that would work if the label was correct... most times when we've had a mistake it was because we got a wrong label... and in Libby's case, they would look the same, just the number/concentration is different.

I have an amazing pharmacist. Well there are two who work the pharmacy I go to. They both know both me and my husband by name and know all the medications I take and my husband takes. If the medication is expensive or not covered by insurance they even work with the doctor to get something that is covered and will work as well for us. They have never made a mistake on our prescriptions either. Oh and for those worried about the medication check out drugs.com. It has a prescription pill identifier which you enter the info on the pill and it will tell you what it is. This is helpful obviously to double check what you are taking is what is actually prescribed and then any side effects to watch for as well and if it interacts with any other meds. You can be more informed that way and know if that weird symptom you are feeling might be a side effect of a medication you are taking

Oh wow, Libby, I didn't know you were taking Lovenox. I had to take it for a month after my surgery last year because of stroke risk and my belly (where I gave myself the shots) was full of bruises and blood under the skin. It stayed that way for many weeks, I never want to take that again. Hope it not too bad for you!

ETA: Yeah, I've known of some pharmacist errors around me too, so I always check the container for my son's med and mine, since we are daily pill takers.

Yep, Stéphanie, I'm in the same boat. Bruised belly and lots of little red marks. I'm pretty used to it at this point. I'll have to do these shots up until 36 weeks pregnant, and then I'll switch to twice a day heparin shots. Then after baby, I'll go back to lovenox for 6 weeks postpartum. Between these, my weekly progesterone shots, and my Rhogam shots I've had to have so far for my negative blood type, I'm getting REALLY used to needles. LOL

Very scary! It's a good thing you caught that.

None of us have routine prescriptions we take, so I always worry that if the doctor or pharmacy messes up we'll have no idea. It doesn't help that the docs always scribble the script so that it's almost impossible to read and that they write a brand name but the pharmacy fills with a generic of a different name. I've had docs write prescriptions for DS that he can't take because of a penicillin allergy, and if I didn't specifically ask (10 minutes after reminding them about it!) no one would have realized. DS's allergy is mild and it probably would be okay, but what if someone had a severe allergy or interaction risk?

Wow, Libby - so glad you noticed!!

We have been very lucky with our medication, (hubby is on a handful of hearts meds every day) our guys always tell us if they are out of stock or have to use a generic etc.

I have found that in a few cases the generic brand makes me sick so have to stick to particular brand, but that's not the chemist's fault

OMG! I am soooo glad you caught it! How scary!!!

How are you doing from the progesterone shots? I ended up having a reaction on my right side...it itched and burned so bad for a few days....but only on my right side. I had no issues on my left side but the shot was suppose to be alternated!!! I wanted to cry every other week!! I felt like a pincushion!!!

You know, Kim, I've heard of others having reactions like that to the progesterone shots, but I had a problem with them yet (I've been doing them for about 10 weeks or so). Did you have them throughout your whole pregnancy, or did they have to stop them due to the reaction?

I did them until 36 weeks (almost 37 weeks) I give those shots all the credit for me going to 37 weeks 6 days with Derek and would do it in a heartbeat again...even if that meant being in so much pain every other week. Our goal was to have a healthy full term pregnancy and avoid the NICU....we did it!!!
